This appendix contains the HDR TID report data where the OPA4H199-SP positive supply voltage (V+) was set to 40V and the negative supply voltage (V-) was connected to ground (GND). The input common-mode voltage (IN+) was also set to 40V, equal to the supply voltage, for this set of HDR testing.
